I bought one earlier this year to use with PS but I have rarely used it. I found it quite tricky to use and no better than my mouse.
It obviously would be better than my mouse for certain tasks if I used it correctly. Like others have mentioned, I really need to take time out to get used to it and find out what it can really help with.

Looks really nice Dave and I think that texture works well.
A version of that would look good with a back support also. I like the style of how the pieces taper in towards the centre.
Is the seat flat on top or have you modelled it slightly sunken? Im not sure if it is modelled like that or the texture gives it that effect.

Maggy I would be really interested to find out how you get on with the Navigator.
Like you I also only have use of one arm, so anything that may help my workflow would be be fantastic.

Edson I agree the mac is a very good alternative and something I personally have been wanting to move across to for quite a while. The thing that always puts me off is that I use Autocad which is not available on Mac. I realise you can run in Bootcamp but I often have Autocad open while using things like PS and SU at the same time.

This is just a basis, you can tweak it as much as you wish, but I was looking mainly at the very good processor Core2 6600 and graphics 8800GT 512mb.
You could save by changing, or taking off the monitor, I added the 22" Ultrasharp which is very good. Also you would have to request an XP install instead of Vista.

If you can let us know the exact mouse you have we could answer you better.
A lot of the mouse software has an option to assign a certain button to a keystroke for different programs.
For example my mouse has the thumb buttons (back and forward default) set in SU to be Shift and CTRL.
A lot depends on the software used for the mouse and the features it has available, but usually I have found most are fully customizable.
