Having trouble with this image any suggestions would be great.
Using "qi medium vispot" settings. Top lights are between 600 and 800 intensity.

Looks like you need more subdivisions to your IR map.
also- ensure the DMC Sample's Adaptive Amount is at 0.85 -

When you say you need more Subdivisons in your IR Map, does this mean where Vray SU says "HSph. Subdivs"?

Yes

How many subdivs are we talking about? I used 90 (default for an interior scene) and it came out blotchy. I had GI off (read somewhere that this could cause blotchyness) so turned it back on, and now rendering with 180 subdivs.
(very new to vray, old hat at SU)

You should check out our presets. Here's the tutorial link: http://docs.chaosgroup.com/display/VRAY2SKETCHUP/V-Ray+Option+Editor+and+Preset+ToolBar#
Try one of the interior settings to get started and work from there.

Do not use emissive materials to light your scene. Some engines can do this, VRay for SU cannot right now. you have to place IES or spots in those cans.
