I am adding a number of features to Door Maker Pro and will put out a Gold version very soon. I have a beta version currently in testing.
I want to change some coding that deals with mullions and panels for cathedrals and arches. I also want to finish off the door jambs portion. Should be released in a couple of weeks.
Here are some pictures of what it can do.

really cool plugin! would be nice if there was a pivoting hinge option (for very large doors).

If you could post a sketchup file with a door and a pivoting hinge I'll take a look at it. It seems to me that you would have to make changes to the door jamb.
Setting the pivot to a non 0 value for the actual door is the easy part. These doors support Interact. However I haven't figured out how to get the second part of a bifold to slide and open in conjunction with the first part just rotating through around 75 degrees.

Rixson pivot hinges are commonly used in commercial construction. The head and floor need to be machined for the pivots and some times the jamb if a center hinge is required.http://www.rixson.com/pivottypes/
