Say what you will but Bill Dance has been one of the more popular fishing show hosts. I don't watch him but (I don't watch television at all) but I am aware of him. I expect that there are a lot of bloopers created on just about any television show produced anywhere. It's just a matter of whether the public gets to see them.

Not always, there was the McDonalds coffee lawsuit lady - I think she was one of the original winners. The requirements may have shifted over time to require that the award be given posthumously.

One of the stories on their front page now is a kid that put an explosive charge between his legs to muffle the explosion - he survived, but got the award.

not death... the actual requirement is "do a service to Humanity by removing themselves from the gene pool" (i.e., lose the ability to reproduce either by death or sterilization in a stupid fashion).
