Can't Make New Template

The "New" button on the paper size window does not work for me, never has.

Where is the paper size window (I must have a worse problem)?
Edit: In the "New from template" command the dropdown window has a "New" tab and a "Recent" tab.

Getting fed up of having to scroll through the same posts each visit?
